Email us
Use the email associated with the account and the subject “Pharmia data request.”

Identify the account
Provide your name, organization, and request type. Do not send patient data.

Verification
We may verify your identity and authority to protect the account from fraudulent requests.

We explain next steps, applicable timelines, and data that must be retained, then confirm completion.

Start a secure request
Do not include clinical or patient information in the email. We will direct you to an appropriate channel if sensitive information is required.
Records controlled by the pharmacy
When a pharmacy is responsible for the requested information, Bento may forward the request to that pharmacy or explain how to contact it. The pharmacy remains responsible for decisions concerning its clinical records.
Required retention
Deletion covers the account and associated data that does not need to be retained. Certain information may be kept when required by law, security, professional obligations, or a dispute. We explain any applicable limitation.
Bento responds to requests within the timelines required by applicable law and confirms when the request is complete.
